A Novel Approach to a Common Problem: After Hours Care of Cancer Patients with Venothromboembolism
In November 2015, our pharmacy began a pilot project to initiate therapy of low molecular weight heparin for cancer patients during times that would otherwise have led the patient to go to the Emergency department. The feedback from patients going through this pilot program has been very positive.
In April 2016, we had the opportunity to present our preliminary data at the International Conference on Thrombosis and Hemostasis Issues in Cancer (ICTHIC) in Bergamo, Italy. We believe that this is viable solution to a current chasm in patient care, and pharmacists can be utilized to improve the patient experience as well as reduce health care costs.
